Appointments are available at the Surgery for patients of the practice on Tuesday from 13:00 pm. Please ask at Reception.
The Suffolk Disability and Health Action Group in partnership with the Ipswich Hospital NHS Trust has produced 'ABOUT ME'. This is to help anyone with a disability and/or health condition provide healthcare professionals with information about themselves in an easy to understand way that reduces the need to keep repeating.
You can use this for hospital and GP appointments, with carers, dentists, opticians etc as well as when you go away on holiday.

Reception will take details from the patient and pass this to the nurse or doctor and if appropriate, a nurse or doctor will return the call and give appropriate advice.
