Social prescribing involves helping patients to improve their health, well being and social welfare by connecting them to community services which might be run by the council or a local charity.

Your practice has partnered with Ipswich Citizens Advice Bureau to provide advice and support to patients when referred by your doctor. This means that, if you need an appointment, basic information will be shared with Ipswich Citizens Advice to allow them to advise you. Both parties have entered an agreement to ensure that your data is kept secure.
