New Patients

To be registered with the Hawthorn Drive Surgery and become a patient of the practice, all new patients are invited to attend for a “New Patient Check”.

Please collect a registration form from reception. Please remember to bring in proof of identity when you return the form to us, such as, a recent utility bill or bank statement showing your name and current address.

New Patient Check

All new patients are invited to attend for a “New Patient Check”. This check is very important as it can sometimes take a while before we receive your full medical records. You will be given a questionnaire and a urine specimen container.

Please bring a repeat medication list with you, if you have one, a urine sample and your completed questionnaire, when you attend your appointment.
